Please let us know if your K-12 student is leaving PACT Charter School.

An Official Withdrawal Request Form must be completed for students who withdraw from PACT Charter Schools. We cooperate with other schools by transferring permanent records to them upon request. If transferring to homeschool, you have 15 days to register with your home school district. 

Please complete this form for each student you are withdrawing. Once a withdrawal form is submitted, it is considered final.
